It could work if it's done wolfenstein style (graphically that is, the movement could still use the modern control scheme of wasd mouse)

Good job, but this wouldn't be as hard as some are saying, VERY HARD yes, but doabe with out a totaly game remake, I have seen it done before. (it would be very limited 3d)

I don't know about that, it could be made from the GMD andi f the only thing changed was view point

Quick list of changes that would have to be made:

Make it 3D
-Add z axis
-rewrite all AI code to accommodate for new z axis
-revamp random room generator to accommodate for new z axis
-revamp physics engine to accommodate for new z axis
Make it FP
-change to FPS control schemes

Personally, I think it would work better as a Third Person than a First person.
One, because I'm biased and prefer third person.
Two, because it would make flip-hangs and ropes more aesthetically constant and pleasing.

Y'know what? You're right. This is starting to seem a lot more plausible now.
It'd be a pain and a lot of effort. But I don't think this qualifies as a pipe dream anymore.

I wonder how the frozen region would work?

Why would it have to have a z axis? I never said it would. the rooms would be quite 2d still. that.. helps a bit.

Althoug MGG's idea of a more Wolfenstein-esque 3D would be easier, I like Keg's idea better. It looks and feels very Spelunky-ish the image I have of it in my head.

Problem being of course how long that would take to do, and that's without taking into accounts all the problems getting the coding to transition or what-have-you slowing it down (as with Taste Like Spelunky and the magical Damsel).

First person (what you and MGG are talking about) simply wouldn't work for a game like this, if it were me (and it may actually be me) I would make it third person like in Sim War V.
