I have some for not seeing the minibosses, some spoilers ahead for those who have not seen at least each area.
-Arachnophobia= no giant spider
-We don't need a bigger boat= no giant piranha (also known by another name)
-Non-believer= no alien boss
-Curse of the what?- no giant mummy
Yeah... Those kind of things would be rather hard to reward. Would it be when you've won the entire game, and you just happened to not see any of the minibosses? That's kind've vague... |

There's no way to have no giant mummy. There's always one on Level 13.

I wasn't sure if any of them were random or not, but it could work if you either never see them on the screen, or just don't fight them maybe.

Maybe just a single one for winning the game without killing or otherwise 'activating' a miniboss. like, no spider drops, no fish charges, no alien shooting at you, no mummy VOMITING FLIES at you.

Yeah, I know I just joined these forums, but I'm a HUGE fan of Spelunky so I want on this.
Anyways...
One could get used to this! - Rescue a damsel for the first time
I'm not a tennis ball! - Get thrown back and forth between 2 yetis...twice.
C-C-C-COMBO MAKER! - Kill 5 enemies in a row without taking damage
C-C-C-COMBO BREAKER! - Kill 4 enemies without taking damage, but take damage on the 5th
OMG!!!! - Get killed by a giant piranha
So close... - Get killed at a level exit
THWOMP! - Get squished by one of those spiky block enemies

Tunneler - Dig to the bottom of a cave using only a pickaxe
A knife in the dark - Kill an enemy using a machete in a dark level without a light source nearby

I AM FREE! - Finished a run with a ball and chain.
Ball + Chain + You = Heavy - try to flip hang but take fall damage from a ball & chain.
Fishing weight - sink in the rushing water level and die because of the ball & chain draggin you down.

That last one wouldn't be very satisfying: you'd actually die from piranhas or frogs or something, so the code would have to be "This achievement is unlocked if you die underwater while stuck with the ball and chain," which could be applied incorrectly. And that second-to-last one isn't anywhere in the code.

I think most of mine are fairly reasonable, but I'm iffy about the death without the shotgun thing. |
