Cry of Fear is a horror mod for HL1 that's been in development for the last couple years. It's made by the same guy who made Afraid of Monsters and this time he's got more people helping him. The biggest deal about this mod is that it's a complete overhaul of Half Life 1 in terms of graphics, gameplay, cutscenes, presentation, atmosphere, enemies etc.
This image seems to be taken with the game's optional film grain setting.
