I think the DLC makes it seriously easier. I get like 2 flying items every run and also there are those eternal hearts. I once got more heart containers than the screen can show.
I already beat it with Isaac, Cain, Samson, Maggie and the "Huh?" character.
It's still incredibly fun though.

I unlocked Cain and Maggie on one run without the dlc and then accidently deleted the game

I imagine it would be much easier unlocking them now with all the white hearts and 2 of diamonds's.

Two separate deals with the devil in one run (might actually be fixed so you could make two deals with the same devil... I dunno).

Oh, is that why I got screwed the first time? I made two deals the day that was added and didn't unlock jack. It was many, many attempts before I was able to do that again.

Also I took this photo a while back, but really had nowhere to show it off. Doesn't really relate to anything, but earlier in that game I was about to give up with only one heart. This shows why you never give up:
