World's hardest game?

My level Death for Spelunky is hard. Out of about 200 tries, i've probably beaten it twice or three times. But that's probably just user error that it's as hard as it is for me.

Also, it's very hard to get an A-rank on every single mission in Sonic Adventure 2 Battle. There's still 4 I never got one on before my disk stopped working

This may sound stupid but Cave Story on Hard

I can't even get past the first boss on 3 HP!

(Although I can get past Red Demon, Misery and the Doctor (2 bosses) with 15 HP, the Blade, Polar Star and Bubbler - it was in a corrupted mod)

I only have the original PC version, so I don't know whether CS+ or CS for other platforms has the hardest difficulty.

Any non-Arkham Batman game, don't know why but all Batman games prior have been impossible.

Honorable mention to Ninja Gaiden (Xbox, not NES), and the only game I've broken a controller over; Chaos Legion for PS2

I guess Kingdom Hearts Re:coded and Kingdom Hearts 358/2 Days counts as this when playing on 1 HP mode.
The former requires chips to be used to connect the "HP Cheat" (that's what it's called in the game, because it's Data-Sora) between the CPU.
The latter requires a certain accessory. Although the game is hard anyway without 1 HP mode
