There are often items underground.  There's a 1/1200 chance of any given block having an item in it.  Particularly in Restless Dead levels, gravestones marked 'Ash' always have shotguns underneath them.

So, no, it's not a glitch.  Just a bonus for the very fortunate (or people with Spectacles).
