The walled-in shop happened in the Mines. Where there would normally be two spaces at the entrance to the shop, there was one space, and in front of THAT space was the block that walled the shop in.

I had a level generated where the shop was walled-in by one block. I carefully tried to bomb my way in by placing the bomb so that the blast would take out the oddly-generated block without touching any of the shop's blocks, but it was apparently within the shopkeeper's zone because he went apeshit and killed me.
It didn't occur to me at the time, but if it happens again I'll take a picture of the screen.

You have to enter the worm from either 2-1 or 3-1, and leave into 2-4 or 3-4. If you enter from 2-1, you can't get the Black Market. Presumably, the Moai Head doesn't spawn in 3-4. Since the Mothership goes up instead of down, you have to replay levels. This means a Moai Head can appear.
Someone correct me if I'm wrong, but in XBLA I'm fairly certain the worm shows up outside of 2-1 and 3-1, and when you exit you simply progress to the next level; for instance, you find the worm in 2-1, go through the worm, and exit into 2-3.

-The ankh achievement didn't trigger the first time I got it. I had to get it a second time for it to pop.
How did you get the ankh? Buy it? Run out of shop with it? Hey, Derek. This happened with my Ankh acheivement, too. I stole it the first time, but bought it the second and third times. Only on my second purchase did I get the acheivement.

Not a bug, but Meat Boy's left-facing and right-facing animations are mirror-images, so his features (black eye, bandaids, missing tooth) switch from his left side to right.

The most reliable way to deal with them is spam the boomerang. It feels great when you walk in the shop and see one for sale, then murder him with it and clear out the rest of the items. Depending on the level generated, you can also pretty easily kill them with boulder traps in world 1. The boulders have some serious momentum in XBLA.

- (addition)You may find small critters scattered around the levels that are harmless and you can pick up (like a small blue frog which acts the same as their bigger counterparts).
Just noticed this: if you're holding one of these critters when you activate an arrow trap in the mines, they absorb all of the damage just as if you were holding a damsel. Handy little guys!
